    Result: PASS
    Joined Microsoft Teams 15 minutes before the exam. Surveyor came 2- 3 minutes late as
    scheduled. Carried out joining formalities as per procedure, room & desk inspection.
    Asked my experience (Oil Tankers) & Trading area.
    Steve Meakings is quite genuine, briefed his requirements very clearly to answer the
    questions in practical way. He gives time to think, prompts if you are going in other
    direction.
    Most Important: NO FIXED PATTERN. He can ask anything from the syllabus.
    I am on Crude oil tanker, voyage from Gulf of Mexico to Japan via Cape of Good Hope (Long
    voyage).
    1. You have to carry 3 grades of crude oil. How will you go about preparing Stowage
    plan? (API, Cargo temperature, draft restrictions, weather during the voyage, MSDS
    if possible, cargo quantity to load, H2S content, how many grades, stresses, Min
    UKC)
    2. Ballast exchange procedure in Gulf of Mexico. (D1, D2, empty refill, overflowing,
    distance from nearest land requirement).
    3. Stability considerations during ballast water exchange. (Change of drafts
    continuously, effect of FSC, over pressurization of tanks, structural stresses, GM,
    bow slamming)
    4. Approach to SBM, procedure & precautions to be taken before operation. (General
    Procedure to be explained)
    5. Your AB on mooring station is not wearing proper PPE, your action? (Asked AB to
    wear his PPE and come back, in the meantime will ask fellow crew about his mental
    and physical state, if he is fine and just negligence, I will report to Master and put
    him on the least risky job during operation, later on verbal warning and enquiry).
    6. How to proceed towards loading of cargo? (Ship Shore exchange, loading rate,
    MSDS, number of connections, chances of cargo contamination etc.)
    7. IG requirement.
    8. PV Breaker working.
    9. How to avoid contamination of cargo, if carrying 2 grades of crude oil? (Single or
    double valve segregation).
    10. Asked to draw and explain flammability diagram.
    11. Gas freeing procedure.
    12. Now approaching south, departure from port, how will you plan your deck
    maintenance jobs? (Planning of Critical jobs).
    13. Ballast tank entry procedure.
    14. Which section of the ballast tank is most hazardous in terms of oxygen depletion? ( I
    told him near bell mouth which is at last and quite inaccessible to carryout initial gas
    test)
    15. Casualty during ballast tank inspection, action?
    16. Casualty is dead? Action as safety officer? (Evidence collection & Investigation)
    17. Categories of Serious Injuries as per MAIB? (He was looking for specific injuries as
    per the list, I didn’t know this answer)
    18. Radio Medical advice in UK waters? (2 Hospitals in UK, which are equipped with
    doctors and will advise you regarding casualty. I do not have much idea bout this.
    Please search accordingly.)
    19. Fire in Deep fat fryer. Your action as an on-scene commander?
    20. The same duty AB now turned up late on navigational watch. Action? (As per
    merchant navy code of conduct)
    21. Preferred channel to starboard buoy in region B, light and rhythm.
    22. Safe water mark, light & rhythm.
    23. Tug & partial submerged tow crossing from port, you are PDV.
    24. RAM crossing from port, you are CBD.
    25. Definition of CBD.
    26. Fog signal of tug & tow, RAM vessel. (I didn’t know this)
    27. RV, own vessel heading 000, 3 blips on bearing 040-050 & 3 blips on port quarter.
    Action? How much alteration will you do?
    28. Shown the picture of an Anchor, asked about all its components.
    29. What is Prohibition Notice? (PSC Notice)
    30. Different leadership styles. (I told him a little bit, not much I remember as I have not
    studied this).
    In the end he said, he was quite convinced with the answers except Fog signals, MAIB &
    Radio medical advice in UK.
    I thank Capt. Sherry for his guidance and doubt clarifications, my partner in Crime
    Yashovardhan who is with me since beginning of this journey and Mr. Bhavin who is running
    this group for helping candidates to achieve their desired goals.
